Serenity Program: The Choice of Satisfaction

The Serenity Program

Monobloc® Silicone SoftOne® + Sizer Hydrogel-CMC Breast Implant Program advantage:

• Automatic and complimentary patient’s registration for the Serenity Follow-up

Program.

The patient’s satisfaction can be measured by aesthetic and medical results: the Serenity Program can guarantee the highest standards to ensure and achieve this result.

Description of the Serenity pack

The Serenity pack is composed of one pair of Monobloc® Silicone SoftOne® breast implants and of one corresponding Monobloc® Sizer Hydrogel-CMC.

The price of this pack is equal to the usual price raised of 150 Euro.

Serenity Program participation conditions

You will, at no cost, automatically be enrolled to the Serenity Program from November 2006, once the patient has confirmed their participation. Receiving the SoftOne breast implant alone does not constitute enrolment. Only first time receivers of breast implants will qualify for the Serenity Program and not those who are replacing their current implants. It is imperative the surgical procedure be performed by a recognized plastics or cosmetic surgical specialist and in compliance with the specific directions for use with

Laboratoires Arion products in the Serenity Pack.

Operational Procedure of the Serenity Program

After the procedure, the surgeon must submit to the patient the serial numbers of the

Implants and the Sizer in addition to protocol papers, which must be duly completed and sent back to Laboratoires Arion to confirm registration to the program. Enrolment may also be completed on the Laboratoires Arion website. The patient will then receive confirmation of their participation to the Serenity Program.

Throughout the duration of the program, it is imperative that the patient retain these documents in order to participate to and benefit from the program.

Details of the Serenity Program

Laboratoires Arion will offer the patients an additional guarantee of satisfaction with a comprehensive follow up to the Serenity Program.

In the 5 years following the procedure, in the case of:

• Loss of integrity of the envelope, causing a rupture which needs to be surgically replaced.

• Capsular Contracture.

Laboratoires Arion will commit to replacement of the affected implant at no cost in addition to offering a 1000 Euro remuneration to be directed with priority to anaesthesia and operating room costs.

Conditions of the Serenity Program

The replacement of the implants and the financial involvement of Laboratoires Arion will not be considered under the following situations:

• Reconstructive surgery.

• Unsatisfied with size or the aesthetic results.

• Rupture resulting from the surgical implantation.

• Loss of integrity of the implant due to a revised cosmetic surgery.

• Loss of integrity of the implant due to capsulotomy (open or closed).

• Replacement of an intact implant on the opposite breast.

• Deformity of the implant or fracture of the gel.

• Competing products to implants from Laboratoires Arion.

• Products from Laboratoires Arion used outside of a Serenity Pack.

In the parameters of the conditions met and the participation to the program, an exchange will only be done with an identical implant. It will not be exchanged with implants of a higher or lower volume.

The exchange of the implant and financial participation due to a qualified revision will only be available once.

To receive validation from Arion Laboratory to qualify for the benefits from the Serenity Program, it is imperative that it be controlled by the technical services of Laboratoires Arion or an expert mandated by Laboratoires Arion.

Within the 5 to 10 years following the procedure, in the case of:

• Loss of the integrity of the envelope.

Laboratoires Arion will replace the altered implant at no cost.

Over 10 years of the surgical procedure, in order to provide the patient with a very reliable product, Laboratoires Arion will apply a preferred lifetime rate to a second implant while providing one free of charge in the event of need for exchange. A copy of the patient card showing the date of the original procedure as well as the serial number of the original implants must be provided.
